Proudly Serving Alabama & Northwest Florida
We don't try to be everywhere — and that's by design. By focusing exclusively on departments across Alabama and Northwest Florida, we deliver a level of care that nationwide providers can't match. We prioritize sending the same screening team to your station each year, building trust and continuity that makes a real difference in the quality of care your firefighters receive. Our staff understands the fire service culture, your team knows our faces, and we're just a phone call away — not a 1-800 number.
Comprehensive Care for
High-Risk Professionals
Our screening program is purpose-built for the unique occupational hazards firefighters face every day. We bring the exam to your station.
Cancer Screenings
Targeted screenings for the occupational cancers most prevalent among firefighters and first responders, using a full suite of advanced ultrasound imaging and a comprehensive lab panel. Early detection is the single most effective tool we have.
Comprehensive Physicals
Thorough head-to-toe assessments focusing on cardiovascular health, pulmonary function, cancer indicators, and overall fitness for duty — aligned with NFPA 1580 standards and tailored to the occupational demands of firefighters and first responders.
Routine Follow-Ups
We don't stop at the exam. Our team follows up at 6 months — or sooner if needed — to ensure abnormal findings are addressed, specialist referrals are coordinated, and your personnel stay on track between annual physicals. Our Nurse Navigator works directly with firefighters to make sure no one falls through the cracks.
Behavioral Health Screenings
Evidence-based assessments for stress, sleep disruption, anxiety, depression, and trauma commonly faced by first responders. Proactive mental health support to keep your team resilient on and off duty.
The Cancer Crisis in the Fire Service
Firefighting has been classified as carcinogenic to humans by the International Agency for Research on Cancer (IARC). The data is clear — early detection saves lives.
Line-of-Duty Deaths
Cancer caused 66% of career firefighter line-of-duty deaths from 2002–2019, per IAFF data.
Higher Diagnosis Rate
Firefighters have a 9% higher risk of being diagnosed with cancer compared to the general population (NIOSH).
Higher Mortality Rate
Firefighters face a 14% increased risk of dying from cancer vs. the general U.S. population (NIOSH).
Mesothelioma Risk
Firefighters have double the risk of mesothelioma, a rare cancer linked to asbestos exposure in structures.
Testicular Cancer Risk
A meta-analysis of 32 firefighter cancer studies found a two-fold excess for testicular cancer.
IARC Classification
Firefighting is classified at the highest carcinogenic level — the same category as asbestos and tobacco smoke.
The Return on Investment
Cost is often the first question departments ask. We get it. But investing in annual firefighter health screenings isn't just the right thing to do — it's a smart financial decision that pays for itself through reduced claims, earlier treatment, and fewer lost duty days.
Get Our Executive SummaryReduced Workers' Comp Claims
Early detection leads to earlier, less costly treatment and fewer extended disability claims.
Lower Healthcare Costs
Early-stage cancer treatment can cost a fraction of late-stage care. The difference often reaches six figures — and that’s before accounting for extended disability, replacement staffing, and long-term benefits.
Fewer Lost Duty Days
Healthier firefighters mean better coverage, reduced overtime, and lower recruitment costs.
Presumptive Law Compliance
Documented screening history strengthens both the department's and firefighter's position under state presumptive cancer laws.

Wellness-Fitness Initiative
Programs aligned with the IAFF/IAFC WFI for holistic firefighter wellness including behavioral health and fitness assessments.
Exam Components
Every VITALExam screening includes a comprehensive suite of assessments tailored to firefighter occupational risks.
Founded on Purpose, Driven by Care
Every first responder deserves care that understands what they face. We built VITALExam to deliver exactly that.
A Team Built for First Responders
VITALExam was founded by Dr. Marci Vitale, a Doctor of Nursing Practice with deep roots in the first responder community and over 20 years of healthcare experience. Our culturally competent team includes nurse practitioners, an exercise physiologist, sonographers, nurses, and a licensed mental wellness clinician — all dedicated to understanding and serving the unique demands of firefighter health.
We prioritize continuity of care by sending the same team to your station year after year. We build real relationships with the departments and firefighters we serve — and the overwhelmingly positive feedback from our clients reflects that commitment.
Vision
A future where every first responder retires healthy — not because they were lucky, but because someone was watching out for them.
Mission
To deliver exceptional, evidence-based care to the men and women who run toward danger — and to build the kind of long-term relationships that make that care more effective every year.
Commitment
Early detection. Consistent follow-up. Ongoing communication. We hold ourselves to a higher standard because the people we serve hold themselves to one too.
More Than an Annual Physical
VITALExam was founded with guiding principles that compel us to go beyond the standard exam. We provide an integral approach that embodies a thorough physical and mental health history and examination.
We promote reduction of occupational risk factors through identification and education, and endow first responders with the knowledge and resources necessary to foster awareness and prevention of occupational-related health conditions.

As a two time cancer survivor, I realize the importance of the services provided by VITALExam. Marci and her team provide an outstanding service that makes the health of our Firefighters a priority. The manner in which Marci and her team provide this service is both caring and professional. They help me ensure the health and wellness of our department, which in turn helps us provide better service to our community.
VITALExam's first responder body scan was truly life-saving — they detected cancer in my body early, giving me the chance to take action before it was too late. Their thorough and professional screening process provided me with answers I didn't even know I needed, and I am forever grateful. This program means everything to me because it not only gave me a fighting chance but also reaffirmed the importance of prioritizing my health as a first responder.
VITALExam has become a vital part of Tuscaloosa Fire Rescue, helping us keep a closer eye on the health of our firefighters. By catching potential health issues early, they not only help prevent serious medical emergencies but also support the overall wellness of our crew. This proactive care ensures our firefighters are always ready for the job while protecting their long-term health and well-being.
The total health and well being of public safety employees are vitally important to Marci and her team. You're not just a client, they treat you as if you're family and your outcome is important to them. They not only run the initial tests, they also follow up and give you a personal information portal that you can access anytime. VITALExam genuinely invests in you and your health journey.
This was our first year using VITALExam, and we couldn't be more impressed. Their team provided exceptional care and professionalism throughout the entire process. From comprehensive physicals to behavioral health support, they went above and beyond to ensure our firefighters were fully taken care of.
VITALExam's focus on the total well-being of first responders truly stands out. They understand the demands of our job and provide the tools and attention needed to keep our department healthy, resilient, and ready to serve our community.
VITALExam has the most knowledgeable and professional staff I've ever worked with. This is the third year our department has used VITALExam, and each year they have identified potentially serious health issues that I was able to address early, before they became major problems. That alone is invaluable.
Beyond the exams, they provide clear, practical guidance on improving overall health. They take the time to explain as little or as much about your personal health as you're comfortable with, never rushing and never talking over you. Quite honestly, they have done more to improve my overall health than my primary care physician.
Highly recommend VITALExam to any department or individual serious about their health.
